All Players (Regular, Spare and New)
Please note that by registering you are confirming your availability to participate in the organization's activities including the BOHA Tournament. Please ensure that you have set aside the appropriate time to help out.
I wish to participate in the activities of the Beaconsfield Oldtimers Hockey Association. I understand and hereby assume all risks and hazards incidental to such participation, including those that may occur during transportation to and from such activities. I do hereby waive, release, absolve and agree to save harmless the BOHA, its executive, directors, sponsors, supervisors, participants and any person transporting me to and/or from any BOHA activities from any claim arising out of any injury, loss, or damages suffered to myself or my property.
As a “Regular Player”, you agree to pay the registration fee of $550 (Fee already includes the $100 BOHA Tournament Tickets). Regular player must submit a $100 deposit no later than May 15th, 2011 in order to guarantee your spot as a regular with BOHA for 2011-12.
